Meet Dr. Betsy Miller, MD

Family Medicine Physician in Richmond, VA

Dr. Miller has been practicing medicine since 2007, bringing nearly two decades of expertise, warmth, and genuine curiosity to every patient relationship. Her commitment is to truly know her patients – not just their medical histories, but the whole person: their health concerns, their families, and the personal wins that make life meaningful.

Dr. Miller believes deeply in a holistic, integrated approach to care, taking the time to explore the root cause of a health concern before reaching for a prescription. Wherever possible, she integrates lifestyle changes, supplements, and evidence-informed therapies alongside conventional medicine to build a plan that fits each patient’s unique life.

“What I love about concierge care is that we have more time with patients,” she states. “More time to meet with them and go through issues that are bothering them, discuss treatment options, and then adjust as needed over the following months.”

For Dr. Miller, that extra time isn’t just a professional luxury – it’s the foundation of good medicine. It means she can get through it all, and build the kind of trusting, ongoing relationships that make real health transformation possible.

Dr. Miller holds a special interest in integrative medicine, seamlessly blending conventional medicine and non-Western modalities like functional medicine, acupuncture, and body work to develop individualized wellness plans. She draws on multiple approaches and therapeutic frameworks, tailoring her recommendations to each patient’s specific needs rather than applying a one-size-fits-all protocol.

Mental and holistic health are additional areas she is especially passionate about. Dr. Miller holds a personal conviction that the mind, body, and soul are deeply interconnected, and that the brain, as an organ, can be actively supported and optimized through the right interventions, not just managed with medication.

Her training in lifestyle medicine informs much of her day-to-day practice. The six pillars (sleep, physical activity, nutrition, stress management, social connection, and substance use management) form the core of her concierge care model. She also brings additional expertise in women’s health, approaching issues through a multi-faceted lens that considers hormonal, psychological, and lifestyle factors together.

Dr. Miller’s Approach to Care

Dr. Miller’s care philosophy centers on partnership, and it begins at the first appointment. She takes the time to get to know each new patient thoroughly, orders comprehensive labs, and often identifies things that may have been missed in prior care. When advanced testing is warranted, such as gut health panels, she incorporates those tools thoughtfully, reviewing and addressing findings together at a follow-up appointment.

In the early months of a new patient relationship, Dr. Miller often schedules more frequent visits, not because something is wrong, but because building a trusting, evolving relationship takes time. She’s tracking progress, making adjustments, and learning how each patient responds, so that care becomes increasingly precise and personal over time.

“Everybody needs a good doctor, good healthcare,” she says of her approach with patients. “Primary care is where it starts, and my mission is to provide excellent care from the beginning.”

She also notes that her approach is sometimes untraditional. She understands that many patients are stretched thin, and believes that creativity feeds the soul, so it’s not uncommon for her to write a prescription to play golf, sit at the piano, or simply spend an afternoon outside, because sometimes what a patient needs most isn’t found through medication.

Above all, Dr. Miller sees her role as a guide, not an authority. Her goal is to empower patients to become the agents of their own health, to help them understand their bodies, their options, and their own capacity to heal. She walks alongside them, offering expertise and recommendations, while recognizing that the patient is ultimately the one doing the work.

This philosophy is captured in the poem she lives by:

Medicine Woman’s Prayer by Sheree Bliss Tilsley

I will not rescue you. For you are not powerless. I will not fix you. For you are not broken. I will not heal you, For I see you, in your wholeness. I will walk with you through the darkness, As you remember your light.

Education

Upon her doctoral graduation from Sidney Kimmel Medical College at Thomas Jefferson University, Dr. Miller completed her residency at VCU Medical Center and went on to serve as an Internal Medicine staff physician there for seven years, providing both inpatient and outpatient care. During that time, she also served as an assistant professor in the VCU School of Medicine, reflecting her commitment to advancing the next generation of physicians.

She subsequently launched her own direct primary care practice, delivering care through an integrative health model, a chapter that deepened her expertise and clarified her vision for what excellent primary care could look like. That vision led her to WellcomeMD, where she has now practiced for six years. The concierge model offered precisely what she had been working toward: longer visits, more resources, and the time to truly take good care of people.

At WellcomeMD, Dr. Miller had the opportunity to learn from founder Dr. Neal Carl, who brought training in functional medicine and the American Academy of Anti-Aging Medicine (A4M) to the practice, a perspective that has further shaped her integrative approach.

“This is like a family business, not corporate,” Dr. Miller says of WellcomeMD, reflecting on what she appreciates about its medical model and how she’s able to deliver care. In fact, many of her patients have been with her since the beginning, and those long-standing relationships are something she genuinely treasures – a testament to the kind of care she has always aimed to provide.

Beyond the Office

Outside of medicine, Dr. Miller is passionate about being the best mother she can be to her two children, ages 8 and 11, and is actively involved in their school community. She also recharges by spending as much time outdoors as possible, reading, making art, and connecting with family and friends.

Currently, she is pursuing certification in botanical medicine through the Chestnut School of Herbal Medicine and is building a garden dedicated to medicinal botanicals, learning the full cycle of growing, harvesting, and working with plants as medicine.

Hear from our patients

I had the most thorough physical I have ever had. None of my prior primary care physicians have ever gone through my blood work results with me as detailed as Dr. Miller took the time to do. I am very excited to finally have a doctor who is trying to get a full picture of my health.
